Nurse Practitioner/Physician Assistant - Clinical Resource HUB

This is a bargaining unit position. Physician Assistant The major duties of this position include, but are not limited to:
Perform evaluations using VA Video Connect for patients calling into the Central Texas Veterans Healthcare System call center seeking clinical advice. Perform initial histories and physical examinations on outpatients. History and physical examinations will mostly rely on the use of CVT technology, but some direct face-to-face visits will also be needed. The supervising physician will review and counter sign at least 10% of all progress notes. Provide ongoing outpatient care for assigned patients. Screen outpatients to determine need for further care. Prescribe medications within defined scope of practice. Order diagnostic studies such as laboratory tests, x-rays, electrocardiograms (EKGs). Review need for an order health promotion and disease prevention activities. Initiate, respond to, and expedite requests for consultations, special tests, and studies. Provide education and counseling of patients and families in preventative care, medical conditions, and use of prescribed treatments and medications. There are backup services available in the local and VA emergency rooms (near the remote clinic site) as well as surgery, psychiatry, and medical subspecialties (affiliated with the remote clinic site.) Patients who need admission to the inpatient wards will be transported to the nearest VA facility when appropriate. No inpatient duties will be required. The physician assistant may sometimes need to work alone, but can request assistance from other medical staff (including other telehealth primary care staff, as well as staff in the emergency room or inpatient service at the nearest VA Medical Center when needed.) Shifts may occasionally deviate from the usual 8 AM to 4:
30 PM shift schedule. The facility is fully computerized and computer skills are required. Special training will be provided to utilize CVT technology. There is minimal administrative work, but the physician assistant will be required to attend the staff meetings, team meetings, some committee meetings, and complete some peer review evaluations. Time will be provided outside of assigned shifts to complete this workup. Advanced Practice Registered Nurse:

The clinical duties include being a primary care provider for a panel of patients in an outpatient clinic, including the performance of histories and physicals, prescription of medications within the defined scope of practice, ordering of diagnostic studies as appropriate, coordination of care within the assigned medical facility as well as specialists and ancillary services in the community, and initiating and responding to requests for consultations, special tests, and studies. (a) Lead and organize delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning; (b) Use advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes, (c) Function as an expert in clinical practice and/or areas related to the assigned roles and responsibilities, (d) Systematically evaluate current practices, and formulates outcomes for groups of patients and/or organizational processes within an area of expertise, (e) Guides, develops, and supports staff from a leadership perspective, and (f) Use professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities. (VA Handbook 5005, Part II, Appendix G6). Demonstrates expertise in leadership at the systems level that is sustainable, experience, and creative approaches to management of complex situations. Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a program, service, facility level, VISN, or VACO. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Must maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. Graduate of a PA training program or a surgical assistant program which is accredited by the ARC-PA or one of its predecessor organizations, AND Possess a minimum of a bachelor's degree from an accredited PA training program or in another health sciences related field from an accredited college or university recognized by the US Department of Education or area current VA employee that was hired as a Physician Assistant prior to September 5, 2014. A Current and continuous certification by the National Commission on Certification of Physician Assistants (NCCPA) is required for employment in VHA As of March 12, 1993, certification by the National Commission on Certification of Physician's Assistants (NCCPA) is a requirement for employment. This requirement does not apply to non-certified PAs on VA rolls as of that date. Grade Determinations:

The following criteria for Physician Assistants must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade. Full Grade. In addition to meeting the basic requirements in paragraph 2, individuals appointed to this grade must meet all the following requirements:
(1) Experience / Education. None beyond the basic requirements in paragraph 2 of this standard.(2) Clinical Competencies:
Clinical Proficiency; Professional Development; Ethics; Interpersonal Skills; Research Intermediate Grade. In addition to meeting the basic requirements under paragraph 2 above, candidates must meet all requirements described in this standard for Full Grade through VA employment or non-VA PA positions with equivalent scope and complexity. Individuals appointed or advanced to Intermediate Grade will have demonstrated the ability to analyze and solve more complex medical problems and meet the following requirements:
(1) Experience / Education (a) 1 year of professional practice as a PA; or (b) Successful completion of a PA post-graduate residency program accredited by ARCPA or by the U.S. Armed Forces; or(c) A Master's degree from a PA training program certified by ARC-PA or in another health-related field from an accredited college or university recognized by the U.S. Department of Education. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
